Off the top of my head, the end of Valkyrie, when Stoffenberg and Olbricht and the like are shot by firing squad.

! Lelouch. I've seen some terrible death scenes, tragic ones and heroic, but this was the worst because it was almost like a Willy Loman-style, "you get fudged, and fudged, and fudged over all your life, then die like a dog" kind of death.
! While Lelouch had the opportunity to plan his death, it felt really unfair and unjustified. The people who benefitted directly from it were betrayers and adulterers, murderers and nationalists. Literally, all of the good quality characters died or were exhiled. Lelouch was tortured for the entire series and denied a happiness.
! All I can cling to is the conspiracy theory that he came back to life as V.V.
Visual Novel (Remember11)
! Takeshi's Bad Ends in Ever17 were horrific, but in spite of how tragic they were, as a man, I found them rather heroic. Certainly they were terrible from his loved one's perspective, but as a guy it seemed like the perfect, "selfish" way to go.
! Now, the cataclysmic avalanche in the end of Remember11, Kokoro's Route takes this cake. It was the product of the series themes, that of predestination. The characters were warned in advance when, and where they were going to die, with a newspaper from the future. Yet, the attempt to use that very knowledge backfired and lead them right to the spot they were foretold they would die at. Kokoro had a mental breakdown, while the other characters were weakened and couldn't even bring themselves up to face the terror.
! When I saw that scene, with the music from my signature playing, I screamed.
! Aloud.
! I screamed, and screamed, and screamed.
! Never before have I felt like "I really screwed myself over" than in that moment. There was nothing smugly heroic about that. The massive, weighing realization that you aren't in control of your life hit harder than the avalanche did. And boy, did it hit deep.

! The whole thing is just so damn painful. Exdeath drops his HP to zero, but he just… keeps... fighting. You can pretty much see it coming and all you can do is keep on fighting as one of the coolest old guys in media goes out in style. Aerith's death is of simply no compare.
! Then there's Tietra in FFT. From her kidnapping to her being slain by her blood brother's COMRADE at the order of her goddamn "brother", I couldn't help but feel sad for her. The whole event leading up to it was terrible all around.

! Honestly, I would count Shirley's death as the saddest. It fits in with that whole "you get fudged and fudged and die as a dog" thing you mentioned, only she did nothing to deserve it. Was Lelouch tortured more than Shirley on the whole? Well yeah, but Lelouch also did some pretty fucked up stuff, so I have to say that he did deserve some of that.

I'm not about to get into a Code Geass debate (cause lord knows I've had it with that show) so I'll close by saying this: ZR was a complete mess that was entirely unnecessary and didn't make sense even by cartoon logic. Not to mention it was implied that he killed countless millions of people while realizing it, only making him even more of an unlikeable douchebag. Oh, and selling your humanity for your dreams; isn't that basically the definition of a villain?
! And I never said that Lelouch deserved everything he got, only that he did bring some of his suffering upon himself, so it's hard for me to feel too sorry for him.

! Judeau. It's hard not to feel sorry for him trying so hard to protect Casca… Especially given what happens to her. He did his best to protect the one he loved but couldn't save her.

! As of now I've only read the first book of the Asoiaf series, so I don't know about the other books. But when I read it, I had no idea of how "easy" main characters could die in this. Ned's death really hit me. I kept thinking "no way, they're gonna save him. Somehow. Any minute now."
There were a lot of deaths that I cried/got teary-eyed about, but they still didn't touch me that deeply. I'm also probably forgetting a lot of great deaths…
I can't really think of any decent one's off the top of my head, but for now I'll pick this.

! 's deaths just fail for me. Ace's death did nothing for me, we barely even knew him. He appeared for like 2 chapters back in Arabasta, why am I supposed to care for a character I don't even know? Also Luffy's face after Ace died was just too cringe-worthy to make me sad. Merry's death scene was just ridiculous imo.

! I know what you mean but I liked the character. Just like most of Oda's best characters a simple existence. He showed us what he was all about not to forget that he was a sweet fighter and Luffys play brother. Oda never tried to justify Ace; it's pretty manly. This is what I am my whole reason for living is to find out if truly deserve to live in this world.

! Boone was kinda sad, but I think Charlies death was the first that really affected me. Just so well done. Sun, Jin and Sayids deaths as mentioned before. Juliets. Jacks, of course. Alex and Eko were more shocking for me, but still, really good. Damn, they were really good at death scenes, weren`t they? Oh, Charlies fake-out death in season 1 got me almost crying, too.

! Joyce`s death. So harsh and real. Uhm, who else died? Tara, yeah that was kinda sad. Anyas shocked me. Oh, Buffys death in season 5.

! The lead writer and director have different opinions, which is good enough for me to say Sunrise has no official stance on whether or not he's alive.
! I would pay money (i.e., buy a DVD!) if it would guarantee us seeing a conclusion to the Geass story, maybe set centuries into the future. Heck, even one generation (starring Ohgi + Viletta's daughter, with Kallen at the same age as Marianne) just to wrap up the nature of Geass and give a satisfying conclusion to both Lelouch and C.C.'s stories.
