I was about to say "it can not be 'erased' as its a brand that was burned into the back" BUT then I remembered what Fisher Tiger did with the ex-slaves that were part of the Sun Pirates.
So why haven't they altered the image? esp. since the Boa sisters feared how other Kuja may react on hearing they were once slaves.
… well besides its a plot device Oda used to introduce the slaves of the Tenryubito.
The Kuja symbol would not cover the 'Dragon's Hoof' (as its called) like the Sun symbol of Sun Pirates does, for the snakes wouldn't cover the three triangles of the mark. But that doesn't mean they couldn't come up with something else, like changing the three triangle to look like three snakes coming from a head, it would also fit the cover story they gave to the Kuja -that the 3 sisters encountered a group of gorgons and were cursed, so none may see their backs.
Its is harder to change a scar from a brand than changing a tattoo. To change the brand would mean another branding, and that could trigger flashbacks, so maybe they never did so because they didnt' want to go through it again.

..... it also could be that the sisters are just not creative enough to think of a way of covering it and never bothered asking the few people who knew (Gorirosa, Rayleigh, Shakky) for help in altering it
