I've always thought people who use the word 'otaku' are Otaku themselves. I've never heard a person say that word outside of the internet in my life, since anime/manga is pretty much God here.

Haha, same here, only that no one says it around here 'cause no one even knows what the hell manga are.
The few people who happen to know/read some manga aren't that much into it, so they wouldn't know what "otaku" means, either.
… either that or everyone is just too ashamed to out themselves.

Well, we use it very freely here. It's used to describe anyone who seems to have avid interest in a particular something. Apparently, it's semi-derogatory. For instance, there was this girl who bought a stack of problem-solving books from the same publisher, and people called her a "(publisher's name)-otaku". But they were just teasing.
Of course, I'm just a student, and moreso I am trapped in school/dorm except on weekends, so what I'm writing here may just be the point of view of a kid who needs to go outside more.
