In the MCU Hulk is due to a formula based on captain America serum.

Oooooh right, because of the line Coulson had in the first Avengers when he's first speaking to Cap.
Which doesn't add up, because the serum worked fine for Emil Blonksy (Abomination), and he didn't get all screwy until he had the hulk stuff added to him, thanks to Mr. Blue.
Actually, no, this isn't quite right.
The creation of the Hulk and Blonksy and Cap is kinda muddied but it's explained in the movies, mostly in Hulk but some light is shed on it in Captain America 1 and Avengers 1.

Bruce Banner was working one one of them, but didn't know it. He thought he was working on radiation resistance. The government just didn't tell him they intended to apply his research to the super soldier project.

Another unspecified attempt to recreate the formula. resulted in the stuff Blonsky used. It was stated to have potential side effects however. Ross said he'd be "Off team" until he straightened out if he displayed any of them.
