The fact that Bach never existed beforehand and had no foreshadowing or indictation of prior existence.
Bach can directly be compared to Dr. Gero in DBZ. He's based on prior elements, piggybacking on an established story, but was not planned out ahead of time.

This could be said for a lot of the stuff happening in this arc. Broken bankai, asauchi, Unohana being the first kenpachi, the list goes on.
And now with the business of nanao having a family heirloom that is a god-slaying zan, as well as Shunsui's zan creating a new spirit with the sole purpose of hiding said zan.

I believe this is the major pitfall of bleach. Kubo bends over backwards to ensure ichigo is this special snowflake. There is no reason for this other than because kubo said so. And then he neglects the plethora of other characters in bleach.
There are also other things fundamentally wrong with the lore in bleach. Byakuya states ichigo can't be a shinigami because only those born from a shinigami can be a shinigami. Then what about all those people from the rukongai who became shinigami? And how did urahara know of this dangerous method of getting a zan? Then there's the bit about only noble families allowed to have children. First, how would you even police a law like that, and second, what about Ukitake? He's not a part of the nobility, yet he's the eldest of five.
U guys realize that talking about "What Kubo did right" will take much less time to do, right? (although it's of course, much less fun).
If we're talking about "What Kubo did wrong", well… it's never going to end and we'll end up with tons of Bleach thread iterations......

And the whole point of that arc was to just get ichigo's shinigami powers back. Only to lose them again fifty chapters later.
I guess Kubo realized in the middle of that arc that Ichigo on that jumpsuit was just ridiculous.

To an extent that's true but Bach pretty much retconned a pre-existing character to be something else entirely which is well beyond what Toriyama did. That'd be like if Dr. Gero were actually Dr. Briefs.

I can imagine some of Bleach casts doing it, especially the insufferable smug ones, but I can't remember the precise moment of said panel's predecessor nor who exactly did it. Possibly because it's just as over-used as Kubo's usual leg shots, close-up mouth shots etc.
But I think I remember Aizen doing it, either after he got bested by Ichigo in his Last Getsuga form, or when he was about to be sealed.

I hate the Zangetsu=Yhwach reveal for many reasons, but one of the biggest is how ridiculous it makes Ichigo vs Kenpachi look in retrospect.
An entire fight dedicated to showcasing the power of the shinigami-Sword bond, where Ichigo only beats sword-mistreating Kenpachi because he learns to respect his sword, and stop just USING it, and he literally wins by teaming up with an avatar of his swo-

Dragon Ball and Yu Yu Hakusho don't have conventional afterlives either. They're pretty much separate worlds that may or may not overlap with the "real" one.

This is true, but I think the issue lies in how specific the concept is played. Dragon Ball has a path shaped like a snake, blue/purple elf-like deities, a large orge running a front desk of souls….it's silly and generalized.
Yu Yu Hakusho has a similar locale - and it's led by a talking baby. (or whatever you wanna call Koenma, dude has a pacifier)
Bleach is too rooted in the Japanese aesthetic, which is pretty cool for a style concept, but since the real world of Bleach is very close to our own....I dunno, I get the feeling that maybe the inconsistencies and inequalities were something Kubo either wanted to explore at some point (I guess the sandal plotline counts???),
or maybe it was a half-hearted attempt to showcase that really, the afterlife is not a peaceful place, portrayed in bright-colored cartoons and storybooks. It can have its own pains, misery...because in the end, that's what life is. Could be me thinking waaay too hard here, but I really sense there's a missing piece or something.
