Close, he pretends to be because you can only be taught the Okama Kenpo martial arts once you have the discover the way of the Okama (Lifestyle/beliefs). Once he has leant his new skills he shall continue to be the normal (ish) sanji we know. With the gained knowledge from the island.

Cringingly horrified would sum it up to a small degree..But yeah, it could happen but I seriously doubt he would continue the Okama lifestyle once he learnt the techniques.
It all seems too similar to the handful of other martial arts plots where the main character has to develop the lifestyle before being able to grasp/learnt the techniques. I could imagine him having to return to the Okama way whenever he wants to use them in a battle though. Mainly for the gags and facial expressions of his opponents when they see him transform into Super Okama Sanj! lol

Sanji will try to escape from Okama island with a ship and will try to meet rayleigh. That's what everyone else will do. What is going to happen to him? I believe he will learn something to make him stronger. They had a lot of trouble with the pacifista. When they will meet again they will definitely be ready for the new world.

2. Look at his right arm: you'll see a raised vein. A raised vein indicates tension. Now, why should a blissful okama feel tense in "paradise?"
3. He's running into the ocean with his shoes off. Most likely, he's gonna try to swim for his life.

And I agree with the Captain and KiShiDo; the facts are there, yet people choose to overlook them. Interestingly, as I've read about them*, the Japanese are generally less accepting of homosexuality than Americans are. Besides that, with Sanji being the 3rd most popular character in many of the official polls, would Oda-san be willing to take a risk so great that could shake a good chunk of the series' fanbase? As an aspiring author, I probably wouldn't do that myself. Not to mention it completely disrupts who he is as a character and it's really difficult to change a mentality someone has grown up with, thus having it ingrained into their personality.

Well, then isn't it a risk to have Okama and transexuals in the story at all? Granted it'd be very risky to change Sanji of all people but if any mangaka could pull it off, it's Oda. That said, I just can't see Sanji not swooning over Nami, Robin and every other pretty women, whether he's in lipstick and a dress or not, lol.

Maybe, Sanji won't become an Okama COMPLETLEY.
Maybe he'll just develop a new combat style in which he uses some form of Okama Kenpo that fits his style of fighting, so that he'll have an alternative to using Diable Jambe (Since, as we've seen in Ch. 512, using it to a degree puts some sort of strain on his legs). But if chances are that the moves Sanji learns seem a little too embarressing or degrading, then he'll just use his new Okama Kenpo as a last resort.

I think its pretty obvious that Sanji has a plan. And for reasons already stated, you can tell Sanji does NOT like what he is going through.
My prediction, is that now he can finally relax and figure out whats going on. During this time he is going to learn about Ivankov, then his relation to Dragon as a revolutionary. And the moment Ivasan gets his butt unpoisened and word spreads that he is at marinford with Straw Hat Luffy, that entire pink kingdom will be in an uproar. If Dragon does show up and he does have members of his revolutionary army stationed on Kamabak Island, you can bet Sanji is going to get swept up in all of this.
If he knows that Luffy is there but still decides not to go, it would probably be because he feels he needs to find Nami and Robin first. Last thing, I can see Sanji learning something from them when it comes to Okama Kempo, but only with his legs. I refuse to think he will ever fight with his hands unless its with kitchen utensils against some food based enemy. But at the very same time, to think that Sanji's own style is at a limit is laughable. Only reason why Diable Jambe hurt him was because Kuma is something even harder than steel according to zoro. Imagine Sanji learns a ranged attack with his legs similar to the death wink or Jimbei's tile punch? I could live with that!

Wait, Okama Kenpo uses the hands too right? Mr. 2 had Douzo Okamai Knuckle and Urabere Swan Budoukai, while Ivankov had that rapid-fire karate chop move. So the possibility of Sanji using the fighting style, or at least part of it, of something he hates seems kind of unlikely to me.

The philosophy of Ivankov and, one would assume, Okama island is that men and women are really not significantly different, or that the difference doesn't matter. This is, of course, the polar opposite of Sanji's personality.
I can see two scenarios: 1. Sanji uses this philosophy to overcome his Kishido, or 2. Sanji learns to pretend his female opponent is really an okama.
I'm afraid of 3: This whole thing was just for some cheap laughs.

I love this. Not that I want to see Sanji beating the shit out of women but it has been getting annoying that he can NEVER KICK A WOMAN. That Calipha fight was bullshit. If he does learn that women and men are equals I would love to see an enemy who has researched the Strawhats pit one of his female commanders after Sanji thinking that he would forfeit and die, then WHACKOW!!! Add Luffy to the mix and it seems like all three of these guys would have dirt on each other. Though I don't think Luffy would be as bothered by it as Zoro and Sanji would, considering that these two were already at odds, anyways… and I don't think they'd have the guts to hold something like that over their captain's head... maybe I'm wrong, but I thought they had more respect for him than that.
