Not sure what you don't get, but if you are wondering at Luffy's logic, I will say there is no logic in grief. He was not looking at it logically. He felt weak because he did not manage to succesfully save his brother. In his mind at that moment nothing else was taken into consideration.

With regards to "queer", "fag", and related slurs, imo, if it's clearly not an insult it's ok. If it's an insult obviously not. if it's too ambiguous, it's better not to use such a term.
Edit: obviously more vulgar slurs are just plain bad in all cases. And we know he meant it as a slur based on the context and our own judgements.
