If we replace DD with Luffy, Lufy had fights with sanji, law then Luffy would be dead from the gamma knife, DD would beat rather easy chinjao, bellamy and who else did luffy fight.
So yeah, no way you can compare the damage luffy got with Doflamingo's damage.
Doula had plenty of time to rest after his fights with Law and Sanji, where he didn't take any major damage. When the birdcage started, he just sit there in his throne.
Hardly comparable.

No, he didn't attempt to block. He did block it. Him being sent flying doesn't change that. Maybe because Luffy kept dodging his attacks? Dofla didn't pierce him through. This came from his own mouth. Luffy also defended himself with Haki. His Haki strength doesn't change, G4 or not. Because it's not relevant. Do you really think that the superficial damage from Bellamy had that much of an effect on his performance? Just because they were both damaged doesn't mean that their damage was equal. That's a testament to his strength and endurance. You seem to forget that Doflamingo's strongest technique is his Birdcage and he never got to use it in his fight because he used it to terrorize the entire island. If it were just him and Luffy, Do flamingo would have killed him easily. Luffy would never be able to break it, and Do flamingo can control the speed it contracts.
Superficial damage? Luffy was coughing blood, just like Doula during Gamma Knife!
Gear 4th completely outclassed Doula. That was shown. He didn't block Kong Gun at all, because the Kong Gun managed to injure him and send him flying in the end. He even coughed up blood after the punch. Blocking can be overwhelmed, you know. It's quite insane that you think it wasn't.
Meanwhile, Doula never hurted Luffy at Gear 4th at all. That's the definition of one-sided. I showed all the panels as evidence. And yes, Luffy dodged all the attacks. That's exactly the point here.
Oh, and he did pierce him alright. You can clearly see in the pages Luffy's blood flying and dripping around. Doula meant a complete piercing through, like he did with Gatz. Still, the fact that Luffy "successfully" defended against Doula's strongest technique (which was also imbued with Haki as seen by its black tips) in the state that he was shows that Doula is not that above him.

Birdcage is not exactly a fighting technique, so it doesn't count. He has to be inside it as the birdcage physics rely on him being the center point of the cage, so he would have to end up fighting Luffy anyway.
Oh, and we don't know if Luffy could break the birdcage. He did manage to break Parasite and Spider Web strings with his Gear 4th and so far, is the only character shown to be able to do so.

Dofla was greaterly injured. Don't know how the cutting of organs can be compared to a few cuts and bruises.
Luffy had internal damage as well. I showed it in the last page all the times Bellamy made him vomit blood (3 times).
And unlike Doula, who did have means to repair himself, Luffy didn't get first aid. So it evens out in the end.
