I think when Kuina fell she did't die, but was just in a comma. And when she walk up, she forgot who she was and just left the village found Smoker, and he called her Tashigi and what happened happened.
Erm… and who did they bury then? O__O
No, I think she died, the way it happened is just surreal that's it.
As for Tashigi's resemblance to Kuina, it's there to make him remember her, his promise and his ultimate goal.

Erm, he already remember even before he met Tashigi. He even fought Mihawk just to honor the promise before he met Tashigi. He doesn´t need Tashigi to remember what´s most important to him or otherwise everone of the Strawhats who lost a precious would require a duplicate person to remind everone of his their goals.

Then, we could say that it's sort of like Oda is mocking Zoro a bit… playing on his patience... =P He goes beserk when he sees Tashigi, he can't stand her... see how he reacted in Alabasta XD
