It's semantics i know but a lot appears to be riding on understanding this phrase and getting this translation correct.

well, given that the translations infer otherwise, i'm not sure trying to accept it based on an unproven theory (the narcolepsy) is really the way to go.

Why not people can name their child as they want to? A lot of people in brazil doesn't give the last name to their child, most name as what they like to. Maybe a fusion of the name of the father and the mother, or the grandmother or whatever. And….. why not in one piece?

My cousin adopted a child who looks so similar to her that few people ever guess she is not the child's birth-mother. It wasn't planned that way – they would have adopted the child even if she looked very different from them -- but it's interesting how it turned out, and how seamless the similarity is.
Ace and Luffy could be like that too : similar enough in looks to pass as brothers, but not actually blood-related.

I agree, biological or not Luffy is Ace's brother. I don't see any reason to infer whether they are biologically related or not even with the different last name.
I think I stated in the spoiler thread, too, but… When I first read the sentence, the feeling I got was also same as sabret00the's: Luffy and Ace aren't blood related. Or at least it's not definite that they are blood related.

Is there an endearing term in Japanese for bro (that can also be used for a sibling) that isn't Aniki or Nakama, but more on equal terms (ie the person you're speaking about is like a brother to you without being blood related).

I think there is, but I can't say so with any authority – my understanding of the Japanese language is very limited!
(I only said "I think so" because I've seen doujinshi translations where Zoro refers to Kuina as his dead "big sister". In these stories, the term "big sister" seems to imply a close relationship, but also a respectful distance -- not a girlfriend, but not just an acquaintnce either...)

isn't it possible that garp left ace with his friend a few years before he did the same to luffy? sounds like garp left luffy with his friend so luffy could train with ace which means his friend was also training ace. like garp said he wanted luffy to be a marine and he probably wanted the same for ace so that's why he left them to train with his friend. obviously garp could not take them with him since it would be way to dangerous and i'm guessing luffy's mother is either dead or abandoned luffy and ace and since dragon is a revolutionary i doubt he would take care of them. i have a feeling that friend was maybe on rogers ship or something and that's the reason why shanks visited that island he had to have a reason for it i doubt it was a coincidence.

As Garp indicated, they may not be brothers. But judging by the way they adress each other, the bond is stronger than blood. (I'm a baka, don't kill me for the cliche'd)… I think they're bro's.

I'm sure all of you know "ojisan" or "jii chan", right? The first one is used as "older man," then second one as "old man." But in a literal sense, it's actually "uncle" and "grandpa." In the same way, "nii chan" and "nee chan" are very, very often used to refer to a boy or girl that's slightly older than you.

We know this:
-Luffy and Ace wound up training together.
-Shanks makes fun of Luffy's handi-cap all the time.
-Shanks seems to be really worried about Ace.
-Garp left Luffy to train with someone.
-Ace calls Luffy "Lil Bro" and travelled across the Grand-Line just to see if he was alright.
Based off of points 2 and 3, it sounds like Shanks is much closer to Ace than Luffy. A real father wouldn't shadow his son (like Shanks' and Luffy's relation) just to make some other boy look better–and we know Shanks isn't like that.
How or where exactly Luffy and Ace trained is quite ambiguous. All that is implied is that Garp gave Luffy one disturbing, Lion-like child-hood, which created fear and animosity. We also know Luffy was always training himself to try and beat Ace--again, Shanks wouldn't scold Luffy like that by pitting him against Ace. In addition to the fact Shanks always avoided or ignored Luffy when he asked him to accept him into the crew; it seems like Oda is suggesting Shanks never directly trained Luffy or Ace, he must have just been the paternly guardian for them and made sure they stayed out of trouble.
So you may indeed be right, ero_seinen. I'm sure we'll learn more when Luffy gets to the pirate's summit (Gets shot by Stephen).
Shanks isn't the one that trained Luffy and Ace.. Shanks left before Luffy started his training and Shanks isn't friends with Garp, Garp's comment shows distaste for Shanks.

Weird, it always occured to me nii-chan and nee-chan were often used for non-siblings as well, sort of expressing a rather close, brotherly bonding. Only example that pops to mind right now, safe from Naruto, is Bleach's Rukia, but I'm positive there are more :)

So they are Brothers for me, Shanks was one year in Ruffys village so he could train him something like his Spirit or anything else

Please don't feed everyone with wrong information when you are not familiar with Japanese.
How one calls his/her bro/sis is totally up to preference and there is no difference among the words used whether or not they are blood-related, except when if s/he is addressing your bro/sis' spouse. Even then, the pronounciation would be the same, and only the kanji would differ.
I don't see where Garp said anything that would imply they are not brothers. Are you guys assuming since he did not call them brothers he is saying they are not brothers? Oda made it pretty clear in arabasta that Ace and Luffy are brothers. Not just with Ace and Luffy calling each other brothers but also through the reactions of the strawhat crew and comments they made about the two.
