In some manga and anime, I've noticed that there are some characters that are just disturbing. They don't even have to be villains, though.
Kimura-Sensei (Azumanga Daioh): While he was somewhat funny. The fact that he was sexually attracted to high school girls just left me disturbed.
Hisoka (Hunter X Hunter)
Orochimaru (Naruto)
Ichimaru Gin (Bleach): I dunno, maybe it's his voice, or his fox face.
Anthrax from Bastard!! He's the first major antagonist, and the manga's subtitle is named after him, Ankoku no Hakaishin, the God of Destruction from Darkness. He (or it) looks like a 2000 feet tall Giger painting, a misshapen biomechanoid monstruosity that lives only to erase mankind from the surface of the earth. Cliché, but damn scary.

I'm sure there are others if I take a while to think about it, but the one that comes to mind right away is Gendo Ikari. (shinji's dad from NGE) I'd have to say he is an exelent "villain" cause just looking at his face, makes me hate him so much. He is such a horrible person. That doesn't happen to much with me and bad guys from TV shows, but Gendo did it.

Maybe he loved his wife, but he also used her to gain power and athority. He screwed around with that computer lady, and her mom. (forget their names) Using them, and abusing them then tossing them aside. He was a god awful father to Shinji. And even though he was a part of NERV, which was fighting for the good of man kind; gendo had his own selfish plans for using the Eva's as was showin in the movie. And The way he treated Rei like a puppet was just sick. And the factf that he was infactuated with her because she was a clone of his dead wife was sick too. That also makes me think of that scene in the movie where he is groping Rie's breast. That was totally unnessisary, even given the circumstances. It was just sick. I mean she's 14 yrs old. He's an old man.

I'd have to say Kurotsuchi whatever-his-name-was from Bleach was a weird ass villain. His Shikai and Bankai were really disturbing, something involving a baby's head, a whole crapload of swords, and poison. Plus his manner of speech and his view on life are disturbed.
Orochimaru places a pretty close second though, when he was laughing at Sarutobi in his new body, I was geniunally freaked out.

I just thought of another one. Legato Bluesummers, from Trigun. That guy just creeped me out. Sure, Knives was in charge, but you don't see much of him. Legato's creepy charicter was more developed.

No, I mean the character Orochimaru is supposed to have an old lady's dialect or someting. You know how Kaku has his old man dialect with his "ja's" and such? Orochi is the gender equivalent of that.

I didn't find the Millenium Earl at all disturbing. Perhaps because he's so over-the-top absurd. Judging from the first volume (all I have):
-The moment when he brings out the akuma on a sprue like a giant model airplane kit is quite funny
-The whole <3 at the end of sentence thing, reminiscent of Chuu, tends to neutralise/humourize the malice in his words.
It's like how Yoichi Hiruma (ES21) would slip from funny to menacing if he was more subtle in presentation. In comparison, Agon in the series is a lot more unnerving because he doesn't have fourteen assault rifles.
Mori Kouran from Flame of Recca (especially after he got the Tendou Jigoku).

Mokuren is pretty fucked up, but that's more about him being a dickhead. Kamui is kind of messed up too, but he's more of a comedic character.
There's definitely a good helping of fucked up people in Flame of Recca.

Berserk has alot of violent disturbing images, but the one which seems to stick out in my mind the most is the image where we seeing Wyald riding into battle with his men while holding up dismembered torsos and body parts of the women and children they had graphically raped in a previous panel. I'm not sure if I should post pics.
Orichimaru!!!! his face… gives me chill's everytime... But after reading the chapter that Sasuke was revealed, made me think othervice, SASUKE is the most disturbing looking villain... EVER cause of the outfit he was wearing... <.<
I'm going to say Seta Sojirou from Rurouni Kenshin. The thing about him that's disturbing is mostly his flashback.
! He helps a criminal (Shishio) that has killed countless people and then brutally murders his family with the wakizashi that Shishio gave him. Also, his entire "The strong live, the weak die." mindset. I'll post a picture of his scary face in his flashback later.

Of course nobody comes close to MPD Psycho's characters when it comes to being fucked up. For ex there's this serial killer in vol 1 who abducts girls, keeps them in a lethargic state with drugs, scalps them, sometimes sever a limb or two and plant flowers in their apparent brain and use them as decoration in his appartment.
When he gets bored of his human flower pots he just rips the flower off, that somehow grew roots inside the cerebral matter…
That may sound retarded but when you read it it's another matter. Ghastly body art at its worst.
