the guy in my Avatar, Gensuru was awsomely evil in the manga and his voice his action his everything translated prefectly
and
the ENTIRE CAST OF ONE PIECE there wasn't 1 character that I have seen so far in 263 episodes with either a wrong voice or wasn't as good
and in August this list will include Pip from the Hellsing OVA's

It's funny, actually. Before I had seen the Alabasta arc in the anime for Kohza's voice I had always thought that the seiyuu who did Trunks in DBZ and Kougaiji in Saiyuki (forgot his name) would be a perfect fit. And lo and behold, when Kohza first spoke, it was him!
Funny you should mention Gensuruu. His voice in the anime ended up being NOTHING like I expected, but I still really liked it. My only qualm was that he sounded SO evil that even when he first appears (and you're not technically supposed to know he's evil yet), it was kind of a dead giveaway.
Though yeah, he's a great actor. More of the HxH cast needs to get more roles…too many of them seem to get very little work, and what they do get are bit parts.

Uruhara from Bleach is high on my list. He has the perfect voice, though I wasn't expecting it to be so deep.
yeah, i'd have to agree that all the One Piece characters translated very well into the anime.
and as for Naruto…. honestly, the anime is better than the manga from what i've seen. i don't really think kishimoto is a great artist.

Yea, I agree with everyone else when they say the OP crew. Also I'd have to say that the Huh Brothers were perfectly done, especially with their 'Huh? Huuuh? HUUUUUH!?' bits.
I think Taki (the guy one, can't remember his full name) from Eyeshield also made a great translation, he is funny in the manga and funny in the anime too, which is just great ^_^
To add to Eyeshield 21, Hiruma is really well done… his evil insane bits are just well done and awesome. Very... well, very Hiruma-like.
In Ranma, I think Ranma (who is Usopp's VA btw) and Ryoga were spectacular. Their voices were just... just... well, lets just say that their roles in Ranma made me fans of both voice actors.
Legato and Vash in Trigun…great voices.

This is more best in recent memory, but still probably one of the best ones I'd be able to think of if I stretched things a bit more. Anyhow, Rapunzel from MAR.
I'm not incredibly fond of the coloring they chose for her outfit, but her voice actress had to be some sort of evil genius to get it down that perfectly.
When we are talking about Eyeshield, I have to say that the voices in Jump festa episode is much better then in the real episodes.
Hirumas YAAA ~ HAAA sounds just as crazy and mad as it should and he even calls people "fucking" in there!
In the other "real" eyeshield anime you can find some good voices and personalitys that fits the character in the manga, but the animations and fillers does still suck bad.
