Was he demoted twice? I am only up to the episode where Luffy gets put on the Wanted list and Fullbody comments that he got demoted. But I thought he didn't get demoted till the Jango sidestory, so is he Demoted twice or just the once?

That´s because it´s a filler thing. in the manga, after Sanji beats him, he doesn´t reappear until the Jango story, and after that, until alabasta

It was never said that he took it voluntarily. It may also be that it was an order for what he did in the court room when he stood up for Jango. Never the less what you say isn´t wrong yet. Just wanted to make clear that it wasn´t officially said that it was voluntarily so both your and my guess aren´t wrong so far.
And as far as the innitial question is concerned. Luffink pretty much summed it up. Fullbody´s appearance in ep 45 was just filler and never happened in the manga.
