Well, here's the thing: Caesar was an extremely goofy, facefault-per-chapter villain. That's actually what made so many of us fans of this fucker: he's a fucking riot. A drama queen, a bad actor, he's facefaulting at every second, has like 7000 different reaction panels (my profile one is probably my favorite). You can clearly see that Oda had a great time drawing this guy. Also, just see how Oda's been portraying him during Dressrosa, and you'll see that Menace and Caesar can't go together in the same sentence.
BUT, the thing is, he did turn those kids into meth addicts. He is definitely a fucking sick asshole. Oda made him into a pathetic loser so we can all laugh at such a person (and hell if we didn't laugh at him), BUT he turned a bunch of kidnapped kids into walking experiments with HEAVY Meth withdrawal.
So, Caesar = Goofy loser. What he did = pretty fucking sick hardcore, had he succeeded.

It is kinda interesting that Ceasar could be so goofy and stupid, yet be probably the most morally disgusting character thus far. Like how Blackbeard's a big, fat jolly guy one second then trying to casually murder or main the next second. There's no off switch with some of these guys.
