Until I read it, I always assumed that Eyeshield 21 was just some sort of bizarre translation of the concept of "Helmet No. 21" (like how some teams have the player's uniform number on the helmet), and that the main character probably goes by his given name on the field.
And Hellsing is not a musical. It would probably make a crack-tacular musical.

I couldn't agree more. When I first heard the name of the show, I didn't exactly think "Oh, this is a comedy with some occasional serious moments, and Haruhi is actually the most carefree character in the show!"
…So yeah, really misleading title. Unfortunately, I can't think of any shows I've seen that haven't already been mentioned, unless it's a show with a fitting title, that is.

Cowboy Bebop: Although they call Spike and the gang space cowboys, it has nothing to do with cowboys, just bounty hunters… except for that one episode with that Cowboy, Andy, and that hardly counts.
Tenchi Muyo: The title is useless Tenchi, yet he's the main character AND the most important (Next to Washu).
Trigun: Misleading at first, because Vash's third gun isn't even revealed until halfway through both the anime/manga, and in the manga it isn't even a gun, it's a power he has.
Hellsing: It has very little do to with the Hellsing family (minus Integra) and more to do with his arch-enemy.
One Piece: The treasure hasn't been mentioned since they entered the Grand Line. And the title itself... who knew what it would be about?

How is Tutor Hitman Reborn! misleading? It's exactly what it is, about a hitman/tutor named Reborn.

Yu Yu Hakusho literally means Ghost Files/Poltergeist Report. It's basically just Yusuke fighting demons, so it's not that misleading.

I always thought it had to do with Kanami's alter ability.
The word "scry" can mean to predict the future. And her ability was kind of like that from what I remember. Well, I think it was kind of different. I haven't seen the series in a few years, so I don't remember exactly. But that's the best I can think of, and also what many other people say.
Although apparently there is something called "scryed" in the manga though.

I know these are old titles, but these two kinda came to my attention.
Those Who Hunt Elves and Bakuretsu Hunters. They don't exactly kill elves in that series, and they don't hunt explosions in the latter. That must have been a popular word to use, like "J" in the 70's.
