One Piece would have a slow decline into unwantedness and there would be a VH1 program about it called: One Piece-Behind the Manga and at some point since Luffy would be a has been, he would appear on an anime version of the surreal life with a beer gut and signs of several facelifts. (Whew)

Im sure the early arcs would be pretty boring taking place with his "Veggie Pirates" and all and the Captain Kuro arc would'nt even appear 'till the middle of the series.

To answer all of the questions:
1. We wouldn't be here talking about One Piece and be talking about god knows what else.
2. Then the series would probably end or alot of fans would quit reading it.
3. Then everyone would be screwed from their injuries, epsically Zoro, Sanji, and Luffy.

The start wouldn't be as wild as the loss of Shanks' arm and the defeat of that Sea Monster. I tell ya, that first chapter was amazing.
What if two of the crew decided to get married? Use your imagination.
